The Forma CPAP Mask incorporates a two-part cushion assembly like those found on the popular FlexiFit 405 and Flexifit 432 masks. The auto adjusting inner FlexiFoam Cushion helps contour the mask to a wide range of faces and promotes optimal frame alignment. The outer silicone seal is frosted to provide a leak resistant surface with a soft, smooth feel against the skin.
SLIDING GLIDER STRAP WITH EASY RELEASE CORD
Forma's Sliding Glider Strap provides freedom of movement and easy pull one step removal. With the ingenious glider strap the Forma's mask frame rests evenly and securely on the face, even when the head rotates from side to side, minimizing leaks.
UNDER CHIN DESIGN WITH CROWN STRAP
The Forma Full Face's Under Chin Design with Crown Strap provides extra stability and maintains the seal, even with movement of the jaw during the night. The Crown Strap works in conjunction with the Under-Chin Design to eliminate mouth leaks and the need for a chinstrap.
MAINTENANCE-FREE ADVANCED AIR DIFFUSER
Fisher & Paykel equipped Forma with an advanced, maintenance free air diffuser that disperses airflow away from bed partners, and minimizes noise from exhaust flow.

.Unlimited leaks
Mask didn’t seal correctly, the leak was really bad and it woke me up 3-4 times a night. I returned it and with the reps help I have to correct fit a seal.

.Best mess out of eight I've tried
This is by far the best mask I've tried so far I tried seven others and they all leaked and were not comfortable this mask is very comfortable and I like that it has a bridge for your chin which keeps it in place better then the other masks I've used this is unique with this company and I hope they continue to make them with this bridge I liked it so much that I just ordered two more. I did have some issues with leakage but I fixed that by tightening the bottom straps and I did have a problem with nose soreness but I loosened those straps that go on the forehead piece and it eliminated some of the pressure and now it's much more comfortable I do use a Band-Aid for this no soreness issue and intend to get pads for the bridge of my nose for future use until my nose bridge gets used to this product I want to say here that all the masks that I've tried give that nose bridge soreness however my doctor says that the bridge of my nose will toughen up soon and I will not need any padding soon and it will not continue to make it sore.

.Does not fit my face !
I had face injuries including a bruised lower lip and wounds to my forehead. Im not a novice at this as i have been using the Simplex model for more than 10 years with no injuries.

The top head piece has an "open" holder (the plastic piece that holds the adjustable velcro strap). Not sure why, when the velcro is adjustable and removable. It is frustrating because the strap keeps slipping out through the opening. This mask is overpriced as well, but I have paid the charge because I like it and it is the best-fitting one I have tried. Just had to put a piece of scotch tape around the opening on the front head piece to prevent the slippage and for that, I wouldn't have expected to pay this much.

.I liked the design. But it was pressing too hard on my forehead giving me headache's.
Good design. But gave me headache's. I liked the under chin design which helps with keeping my jaw from dropping during sleep. I didn't like the quick release **** the harness/strap.

At one time this was a superb mask then this last year the mask went to very poor performance. The elbow that connects to mask to the supply hose will no longer turn freely and will wrap around the neck waking you up, and whats worse is the manufacturer could care less.

Over the past several years, I have tried every mask available on the market for my sleep apnea. Although there will never be that "perfect" fitting mask in my opinion, the Forma full face mask comes closer than any brand I have used. This is the first comfortable and best fit mask that I have found since wearing a CPAP mask. It is so easy to take off. Previous masks I struggled to get on and off. I usually end up just pulling it over my head causing the head gear to stretch out and fall apart. I love this mask.

Tried just about every mask there is, nothing compares to the old Resmed mask i had; including this one. Mask just doesnt fit right and worse than that it wont stay connected to the hose. I use 10 lites of oxygen with my mask which makes it very important the mask stays connected to the hose.

It seem all full mask makers cannot make a decent XL mask. This one fits well but the nose section is not wide enough for a XL face. It pinches the nose and makes it hard to breathe through the nose.

I tried the a few nights and must constantly adjust it trying to stop the leaks. It wakes me all during the night and the only way I can get it to stop leaking is to wear it very uncomfortably tight.

.One of the best for sure
I chose this particular mask because I am both a nose and mouth sleeper. I was finding that my mouth was falling open below my previous mask so was looking for something under the chin. This mask is comfortable and easy to pull on and off. My only gripe is that it does leak and not always from the same spot. I find myself constantly adjusting straps. However these are not the huge mask farting leaks like other masks. I feel like at times, the straps are stretched as far as possible and actually dig into the back of my neck they are so tight. If I could get past this, the mask would be perfect. I think if you sleep on your back most of the time this would be absolutely perfect but as a side sleeper, I will continue my nightly adjustments. The adjustments are still worth the trouble as it is the best mask I've used so far.

.Finally a mask that fits!
I have been going through sleepless nights with all the mask changes trying to find a mask that fit my needs. The Forma Full Face CPAP mask fits the bill where three other masks have not. First tried the ResMed Quattro, then ResMed Liberty, then Hybrid. None worked with the pressure my doctor prescribed. In fact, the Quattro actually wore a raw spot on the bridge of my nose in my efforts to seal the leaks. Now,the Forma is nothing but comfort compared to the rest.
